Eligibility

Under Waiver Reimagine, personal supports and adult companion are streamlining into one service called individualized home supports without training as of January 1, 2021 (or upon federal approval).

For more information about individualized home supports and the transition throughout 2021, please visit the Department of Human Services (DHS) manual page for individualized home supports and the FAQ section of the Waiver Reimagine website.

Please note: Waiver Reimagine simplified service changes are only for the disability waivers - Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI), and Developmental Disabilities (DD) - and will NOT affect the Elderly Waiver and its use of adult companion service.

Fees

If you have waiver or Alternative Care Program funding, the service plan (based on an assessment of your needs) determines which services are funded through your waiver.
