Mental Health Case Management is the coordination of services designed to serve people of all ages with serious and persistent mental illness. Plans for mental health services are customized for each person's needs and designed to help people live with as much independence within the community as possible.
Case managers work closely with individuals and families to help to access needed supports and services to maximize independence, mental health and community integration.
Carlton, Lake, and St. Louis Counties contract with Human Development Center (HDC) to provide case management services to adults and children with serious mental health issues.
Accept many types of insurance, including Medical Assistance (MA) for covered services.
No one is denied services due to inability to pay. Each location has a sliding fee scale available based on your income for people without insurance.
